MBBS in China takes six years at most English-taught universities: five academic years followed by one year of clinical internship. That internship year is part of the six, not extra. The structure matches what the Indian Embassy in Beijing describes (five years plus one year of internship), and most programmes in our directory list the same pattern. Some universities add a Chinese-language foundation year inside the six years, and a few run shorter or longer plans, so always read the programme page of the university you apply to.

This guide explains MBBS in China duration year by year, what is taught in English, where Chinese becomes necessary, how the plan compares with the 5-year MBBS in Pakistan, and what to ask a university about hospital access before you pay a deposit.

How many years is MBBS in China?

For international students in English-taught programmes, the standard answer is six years. The Ministry of Education issued interim quality-control provisions for English-medium undergraduate medical education for international students in 2007. A 2025 peer-reviewed paper on these programmes describes the degree as a six-year English-medium programme with the sixth year given to rotational internships. Individual universities write their own curriculum within that frame, so the details differ.

Here is how the six years are usually labelled in our university directory, per our directory:

Pattern in the directoryWhat it means
6 years (5 academic + 1 internship)The most common listing. The internship is year six.
6 years (1 Chinese + 5 MBBS including internship)Year one is mostly Chinese language and foundation courses, and the remaining five years contain the medical course and the internship.
6 years (Year 1 Chinese, Years 2 to 5 clinical, Year 6 internship)Same idea, written year by year.
5 years including internshipAppears for dental programmes and for one or two entries. Check what the degree is called and whether it is recognised for your purpose.

If two universities both say "six years", the content of those six years can still differ. Ask for the programme plan (the teaching schedule) in writing, not just the brochure line.

Is the internship year included in the six years?

Yes, at the universities that list "6 years (5 academic + 1 internship)". The internship is the final year, taken in the university's affiliated teaching hospitals. You still pay tuition and hostel fees for that year at most universities. In the fee tables in our directory, year six normally has the same tuition line as the earlier years. Always confirm the current fee with the university. For the full money picture, see our MBBS in China total cost guide.

One older but useful official example: a Jiangsu University international office notice from 2017 describes the clinical internship in China as 48 to 52 weeks, with set rotations. It listed 12 weeks of internal medicine, 12 weeks of surgery, 6 weeks each of obstetrics and gynaecology and of paediatrics, and 2 weeks each in subjects such as ophthalmology, ENT, dermatology, infectious diseases, emergency and radiology. That notice is old and is one university's plan, so treat it as an example of how an internship is built, not as a rule for every university.

MBBS in China year by year

The peer-reviewed paper on international MBBS programmes in China describes the general shape like this: year one is liberal arts and natural sciences, years two and three are preclinical study in basic medical sciences, years four and five are clinical courses with bedside teaching, and year six is the rotational internship. Many universities follow that shape. Some start clinical teaching in year three or four, and some programmes with a Chinese foundation year move everything one step later.

YearTypical contentWhere you study
Year 1Chemistry, biology, physics foundations, medical English, Chinese language, culture and ethics. At some universities this year is mostly Chinese.Main campus
Year 2Human anatomy, histology, embryology, physiology, biochemistryMain campus labs
Year 3Pathology, pharmacology, microbiology, immunology, medical statistics, introduction to clinical skillsMain campus, early hospital visits
Year 4Internal medicine, surgery, diagnostics, imaging, clinical skills, bedside teaching begins or deepensTeaching hospitals
Year 5Obstetrics and gynaecology, paediatrics, neurology, psychiatry, infectious diseases, community medicine, forensic medicineTeaching hospitals
Year 6Rotational internship through the major departmentsAffiliated hospitals

Treat the subject placement in this table as a typical pattern, not a promise. Subject names and the year they are taught change between universities. The paper I used for the general shape is cited in the sources below.

Years 1 to 3: the basic sciences

This is classroom, lab and dissection-hall work. If you did FSc pre-medical, much of year one feels familiar, but the pace is faster and tests are frequent. Most Chinese universities run a semester system with end-of-term exams and make-up exams. Failing too many subjects can delay your promotion to the next year, and at some universities it can delay the start of the internship. At Jiangsu University, the 2017 notice said students could carry no more than 18 credit hours of make-up or relearn exams into the internship.

Years 4 and 5: clinical teaching

Here you move into the teaching hospitals. You attend lectures, bedside teaching, case discussions and short department postings. How much real patient contact you get depends on the hospital, the size of your class, and how the university handles international students. This is the part you cannot judge from a brochure, which is why the questions at the end of this guide matter.

Year 6: the internship

The internship is a supervised rotation through departments. Universities usually require you to have passed all earlier subjects before you begin. Some also require a Chinese-language level or a short medical Chinese course first. The Jiangsu notice, for example, required HSK level 4 with a score of at least 180 and a one-month medical Chinese course before the internship.

When do clinical rotations start in MBBS in China?

Most programmes introduce hospital exposure in year three and run proper clinical teaching in years four and five. Peking University's published pages show why you must read each university's own plan: its English admissions page says the basic medical period ends in year four, while its Chinese-language page says year three. That programme is also taught in Chinese. So when someone gives you a flat "clinicals start in year 3", ask which university and ask them to show the plan.

What is taught in English and when is Chinese needed?

The classroom teaching in an MOE-authorised English-taught MBBS is in English. The Indian Embassy in Beijing notes that, under the Ministry of Education's 2019 notification, only 45 listed universities may admit foreign students to English-taught clinical medicine programmes, and that programmes taught in Chinese or bilingually are not permitted for foreign students. Our MOE-listed universities list shows which universities in our directory carry that listing.

Chinese is a different matter. Patients in the hospital speak Chinese, and the same embassy advisory says learning Chinese is mandatory for clinical sessions, up to HSK-4 level, and that students who do not reach it may not be awarded the degree. The 2025 paper cites a survey where students said oral Chinese skills were critical for effective clinical clerkships. A university's own rule can be stricter or lighter than that, which is why the HSK rule is listed per university in our directory.

The patterns we see in the directory are these, per our directory:

  • HSK-4 required by the end of year one, or Chinese language training from year one.
  • No HSK during the early years, then HSK-4 before the internship or before graduation.
  • HSK-3 by the end of year one, HSK-4 before the internship, HSK-5 before graduation at a few universities.

Read the exact rule on the page of the university you are considering, and see do you need Chinese for MBBS in China and the HSK exam guide for levels, scores and how to prepare. The short version for a parent: lectures are in English, but a student who cannot talk to patients in basic Chinese will have a weak internship and may be stopped from graduating.

Do I need Chinese for ward duties?

Yes, at a practical level. You do not need to speak like a native, but you need enough medical Chinese to take a short history, understand instructions from the ward doctors and read basic notes. Start Chinese in year one, not in year five.

MBBS in China vs Pakistan MBBS years

The MBBS in Pakistan is a five-year programme followed by a one-year house job, which is the supervised foundation year. In China the six years include the internship, so the comparison is less different than it looks.

On the entry test: since PM&DC public notice No. Exam-59/2026/350 of 5 October 2026, MDCAT is not required for Pakistani students seeking admission to foreign medical institutions. MDCAT still applies if you want a college inside Pakistan. Read our summary of the PM&DC notice on MDCAT. Other conditions reported earlier in 2026, such as an FSc percentage and a registration portal step, are not confirmed after that notice, so confirm them on pmdc.pk.

Does the MBBS in China have the same syllabus as Pakistan?

No. The core subjects overlap, because anatomy, physiology, medicine and surgery are the same everywhere. But the teaching plan, disease patterns, clinical exposure and exams differ. Chinese universities also add Chinese language and culture courses. A graduate returning to Pakistan has to pass the registration exam, which is based on the Pakistani expectation, so plan extra revision in the final years.

Internship in China or Pakistan: where can you do it?

This is the question families most want answered, and the honest answer is that it depends on two things: the rule of the university that grants your degree, and the rule of the Pakistani regulator when you return.

First, the university. At most universities in our directory, the internship is listed as part of the six-year programme in affiliated hospitals in China. Some universities allow students to do the internship outside China if they meet conditions. The Jiangsu University notice, for instance, allowed an internship outside China for students with no make-up exams and a hospital acceptance letter from a teaching hospital with over 500 beds. That is one university's 2017 rule. Do not assume your university offers the same option, and ask in writing.

Second, Pakistan. A PM&DC rule reported by a Pakistani education news site says no foreign graduate may start a house job in Pakistan without provisional registration, and that graduates of recognised foreign institutions can apply for provisional registration directly while others must first pass the National Registration Examination (NRE). The report did not give a date, so confirm the current rule on pmdc.pk. Our PMDC registration guide walks through the steps, and our guide to checking PMDC recognition of a Chinese university shows how to verify it yourself.

Does the internship year count towards the house job?

Not automatically. Pakistan can ask for a house job after registration even if you completed a hospital internship in China. We have not found a published PM&DC rule that credits a Chinese internship against the house job, so do not plan on skipping it. Ask PM&DC directly.

House job after MBBS from China

If you plan to work in Pakistan, the sequence after graduation is usually: collect your degree and transcripts, get the documents attested, apply to PM&DC for registration, sit the NRE if required for your university, then complete the house job. You can read the detail in the post-MBBS PMDC registration guide.

A word of warning about the NRE. In the December 2025 session, reported figures were 333 passes out of 2,154 candidates from China, roughly 15.5 percent, against about 21 percent overall. These are figures from Pakistani news reports, not our own data, and no 2026 result for China was available when this was written. The practical lesson is to study for the registration exam during your academic years, not after graduation.

What to ask a university about hospital access

The same six-year label can hide very different clinical training. Before you pay, ask these in writing and keep the replies:

  1. Which hospitals will I train in, and how far are they from my hostel?
  2. In which year does clinical teaching start for international students?
  3. Do international students rotate in separate English groups or join Chinese-speaking classes?
  4. How many international students share one ward group?
  5. What exact HSK level is required, and by which year?
  6. Is there a medical Chinese course before the internship, and is it included in the fee?
  7. How many weeks is the internship, and which departments does it cover?
  8. Can the internship be done outside China, and under what conditions?
  9. Is the sixth year charged at the full tuition rate?
  10. Can you show a sample programme plan and a sample internship schedule?

A university that answers clearly and specifically is a better sign than one that repeats the brochure. If the answers are vague, treat that as information.

Check recognition before you check the curriculum

A good curriculum does not help if the degree cannot be registered where you plan to work. The Indian Embassy advisory says authorised universities appear in the WHO World Directory of Medical Schools. For Pakistan, write down the university's name exactly and check it on pmdc.pk and on search.wdoms.org. Our directory shows an MOE-listed flag for each Chinese university, and a flag showing whether it is listed as PMDC-approved in our directory. Both flags are a starting point only, and the final check is the regulator's own list. If you plan to practise outside Pakistan, check ECFMG's medical school policy as well, because it sets conditions on a school's graduates such as government confirmation that the degree meets licensure requirements in the school's own country.

You can browse the full list in the MBBS in China university directory. For eligibility rules before applying, see MBBS in China eligibility for Pakistani students, and for the complete route from application to arrival, the complete MBBS in China guide.

Frequently asked questions

How many years is MBBS in China?

Most English-taught MBBS programmes in China run six years: five academic years plus one internship year. Some universities place a Chinese-language foundation year inside the six. Dental programmes in our directory are usually listed at five years. Check the programme page of your chosen university, because the length is set by the university within the Ministry of Education framework.

Is the internship year included in the six years?

Yes, at the universities that list "6 years (5 academic + 1 internship)". The internship is the sixth year and is normally paid for at the university's tuition rate. Confirm the sixth-year fee and the internship rules with the university before you pay, because some require passed subjects and a Chinese level first.

What subjects are taught in each year?

Typically, year one covers foundation sciences and Chinese. Years two and three cover anatomy, physiology, biochemistry, pathology and pharmacology. Years four and five cover medicine, surgery, paediatrics, obstetrics and other clinical subjects. Year six is the rotation internship. The exact placement changes by university, so ask for the programme plan.

When do clinical rotations start?

Hospital exposure usually begins in year three and proper clinical teaching in years four and five. Some universities start earlier and some later, particularly those with a Chinese foundation year. Ask the university in which year international students enter the teaching hospital and how large the ward groups are.

Is the internship done in China or can I do it in Pakistan?

At most universities the internship is done in China at affiliated hospitals. Some universities allow an internship elsewhere under conditions, but you must get that in writing. Pakistan has its own rules on provisional registration and house jobs, so confirm with PM&DC at pmdc.pk before planning an internship outside China.

Does the MBBS in China have the same syllabus as Pakistan?

No. The core medical subjects overlap, but each country sets its own curriculum, exam style and clinical exposure. Chinese programmes also include Chinese language courses. Because Pakistan requires a registration exam for foreign graduates, you should revise Pakistani-style questions on your own during the academic years.

Do I need Chinese for ward duties?

Yes. Lectures are in English, but patients speak Chinese. The Indian Embassy in Beijing advisory says Chinese up to HSK-4 is mandatory for clinical sessions. Many universities set HSK-4 before the internship or graduation. Start learning in year one so your internship is useful, and see our HSK guide for levels and scores.

Is MBBS in China six years or five years?

For English-taught MOE programmes it is normally listed as six years, because the internship year is counted. People who say five years usually mean the academic part only. The comparison with Pakistan's five-year MBBS plus a one-year house job gives a similar total of about six years.

The short version on MBBS in China duration

Plan for six years: five academic years, one internship year, and a Chinese-language requirement that shows up at the hospital stage. Match that plan to the specific university, get the programme plan and the HSK rule in writing, and verify PM&DC recognition on pmdc.pk before you pay anything. Fees and details on this page come from our directory, so confirm the current fee with the university.
