If you are going to China for MBBS, you will meet HSK sooner or later, even on an English-taught program. Under China's standard for English-taught MBBS, your Chinese must reach level 4 before clinical internship. Universities enforce this: no HSK 4, no internship, and without the internship there's no degree. Students who ignore Chinese until Year 5 are the ones who end up stuck.
HSK is also changing. The new HSK 3.0 launches worldwide on December 13, 2026 and becomes the regular exam from 2027. This guide covers both versions, the sections and passing scores, the actual HSK rules at Chinese medical universities, and where to take HSK in Pakistan.

What Is HSK?
HSK (Hanyu Shuiping Kaoshi, 汉语水平考试) is China's official Chinese proficiency test for non-native speakers, run by Chinese Testing International (CTI). You register and get your results on chinesetest.cn. It is to Chinese what IELTS is to English.

Why HSK Matters for Pakistani Students
- Your MBBS internship and degree: most English-taught MBBS programs require HSK 4 before clinical internship or before graduation. Miss it and your internship or degree is delayed.
- Learning on the wards: doctors may speak English, but patients usually speak only Chinese. As Shandong University puts it, mastering Chinese lets you communicate with patients and gain the most clinical knowledge and skills.
- Scholarships: Chinese Government Scholarship (CSC) applicants for Chinese-taught Bachelor's programs need HSK 3, and HSK 4 for Chinese-taught Master's and PhD. A higher level makes you more competitive.
- CSCA: Chinese-language majors with a valid HSK 4 can skip the Chinese paper at some universities.
- Careers and daily life: Chinese makes six years in China far easier and helps in China-linked careers, including CPEC-related work in Pakistan.
HSK 2.0: Levels, Sections and Passing Scores
All regular HSK sittings before December 13, 2026 use HSK 2.0.

Allow about 5-10 extra minutes on test day for filling in your personal details.
Passing percentage: 60% of the total at every level, which is 120/200 for HSK 1-2 and 180/300 for HSK 3-6. Each section is scored out of 100, and pass or fail is based on your total. We found no official per-section minimum.
Watch the wording "HSK 4 (180+)". Many universities write their rule this way. It means you need at least the passing score, so a certificate at the right level with a failing total won't count.

HSK 4 means you can handle most daily situations and follow simple conversations. That's why it's the MBBS benchmark. Clinical Chinese is still a step up: a patient describing symptoms at speed is very different from an exam recording, so keep learning after you pass.

Cumulative word counts. HSK 3.0 figures are from the late-2025 syllabus as compiled by secondary sources, so check the official syllabus on chinesetest.cn. Older articles quote higher HSK 3.0 numbers (for example 500 words at level 1). Those come from a 2021 draft that was never used.
What this means: a new HSK 4 needs about 2,000 words, compared with 1,200 for the old HSK 4. The same level number is harder in HSK 3.0.
Other Changes
- Speaking matters more. In the 2026 trial, levels 3-6 required candidates to register for the matching speaking test at the same time.
- New prep material. Old HSK 2.0 books won't fully cover HSK 3.0. Use the official syllabus and mock tests.
Is My HSK 2.0 Certificate Still Valid?
Sources that cite CTI's transition rules say HSK 2.0 certificates you already have stay valid and recognised, but we haven't found an official CTI statement yet. For university admission, the 2-year score validity applies anyway, and each university decides how it treats HSK 2.0 levels after the switch. Ask your university before assuming an old HSK 4 equals a new HSK 4.
Should I Take HSK 2.0 or Wait for HSK 3.0?
- Need a certificate soon? Take HSK 2.0 before December 13, 2026. Most study material covers it.
- Starting from zero? Study for HSK 3.0. That's what you'll sit from 2027.
- Already on MBBS in China? Check which version and level your university's rules name, and whether they will accept HSK 3.0 at the same level.
HSK 7-9 (Advanced)
One combined exam of about 210 minutes and 98 questions. It covers Listening, Reading, Writing, Translation and Speaking. You get five section scores and an overall level (7, 8 or 9) based on your results. Most MBBS students won't need it.

Some programs ask for HSKK alongside HSK. For example, the Confucius Institute at the University of Karachi lists HSK 4 (180/300) plus HSKK Intermediate (60/100) for its program completion.
HSK and MBBS in China
The Rules Behind HSK 4
- The general rule (2018): the Ministry of Education's quality standard for international students (教外〔2018〕50号) requires level 4 Chinese by graduation for foreign-language-taught Bachelor's programs, and level 5 for Chinese-taught ones.
- The MBBS rule: China's quality standard for English-taught Clinical Medicine goes further. It requires students to learn basic Chinese and medical Chinese and to reach level 4 before clinical internship (临床实习前汉语能力须达到《国际汉语能力标准》四级水平). Chinese and medical Chinese are compulsory courses throughout the pre-internship years.

For Chinese-taught Clinical Medicine the bar is higher. Shandong University and Jilin University require HSK 5 (Jilin: 180+). Rules come from each university's own notices and can change, so check yours. Our China university pages list each university's HSK rule.
What Happens If You Don't Reach HSK 4?
- Internship blocked. At universities like Nantong, you can't start clinical internship without HSK 4.
- No degree certificate. Wenzhou University's rule: students who don't meet the HSK graduation level cannot apply for their degree certificate.
- No extra time. Wenzhou also says you can't extend your study period just to retake HSK.
- A late fix is possible. At Wenzhou, if you pass HSK after leaving but within the maximum study period, you can still apply for the degree. Nantong sets a maximum of 10 years for its 6-year MBBS.
- Dismissal: we found no university rule that dismisses students for HSK alone. The documented consequence is a delayed internship or degree.
For a Pakistani student, a delayed degree means a delayed PMDC process and a delayed career. PMDC doesn't ask for HSK itself, but it needs your degree, and the degree needs HSK.
Why a University That Asks for HSK 4 Early Can Be the Better Choice
Many students avoid universities that want HSK 4 in the first year or two. In practice it often works in your favour:
- You need it anyway. The MBBS standard requires level 4 before internship everywhere. An early deadline just gets it done while you have the most time.
- Your clinical years get better. On Chinese wards, students who speak Chinese talk to patients, follow rounds and read records. Those who don't mostly watch.
- No internship or degree block later. Early HSK removes the biggest risk in Years 5 and 6.
- It's often a sign of a well-run program. Building Chinese into the early years is exactly what the MBBS quality standard asks for.
- It's easier while you're living it. Years 1-2 have lighter clinical demands, and you're surrounded by Chinese every day.
Before you enrol, ask each university exactly what happens if you miss its HSK deadline.
The Medical Chinese Test (MCT)
MCT (医学汉语水平考试) is an official test of clinical Chinese. It was developed by China's Center for Language Education and Cooperation, CTI and five leading medical universities, and first held in December 2019 for 850 clinical medicine students from 46 countries.
- Sections: Listening, Reading and Writing, each out of 100 (total 300)
- Levels: 100-149 = MCT Level 1, 150-199 = Level 2, 200-300 = Level 3
- What it shows: you can talk with patients, understand medical records and join clinical case discussions in Chinese
MCT isn't required everywhere, but some medical universities use it. It's a strong addition for students aiming at internship or practice in China.
CSC-Funded MBBS
CSC-funded MBBS is Chinese-taught. CSC Chinese-taught Bachelor's programs usually include a one-year Chinese preparatory course, and you must reach the required HSK level before starting the degree. The CSC baseline for Chinese-taught undergraduates is HSK 3, but medical programs often want more. Shandong University asks for HSK 5 for Chinese-taught Clinical Medicine. Taking HSK in Pakistan
HSK runs in several Pakistani cities through Confucius Institutes and authorised centres:

Test modes: paper-based and internet-based at centres. The home-based online HSK has also been offered to students living in Pakistan. Use the test-centre search on chinesetest.cn to see current centres, dates and modes.
Fees vary by centre. In 2024, reported fees were PKR 1,600 for HSK 1-2, PKR 3,500 for HSK 3-4 and PKR 5,500 for HSK 5-6. In late 2025, one centre listed PKR 2,500 for its own students and PKR 5,000 for external candidates for HSK 1-2. Expect changes with HSK 3.0, and confirm with your centre.
Registration and Results
- Create an account on chinesetest.cn.
- Choose the test (HSK level, plus HSKK if needed), mode, centre and date.
- Enter your passport details exactly and upload a photo.
- Pay the fee (online or at the centre).
- Print your admission ticket and bring your original passport on test day.
Results usually take about 2-4 weeks. The HSK 3.0 trial took about 6 weeks. You get an electronic score report on chinesetest.cn, and paper certificates come through your test centre. Leave plenty of time before any deadline.

- Start with pinyin and tones, then characters. HSK 3 and above test characters.
- Use the official word list for your version (HSK 2.0 or 3.0).
- Take official mock tests under timed conditions.
- For MBBS students, add medical Chinese early: body parts, symptoms and hospital phrases. Consider MCT later.
- Start before you fly. Even HSK 1-2 before arrival makes Year 1 much easier.
Common Mistakes
- Leaving HSK to Year 5 and blocking your internship or degree.
- Assuming English-taught MBBS means no Chinese.
- Passing at the right level with a failing total when your university asks for "180+".
- Studying only pinyin and hitting a wall at HSK 3.
- Using HSK 2.0 books for an HSK 3.0 exam.
- Letting your score expire. For admission it's valid for 2 years.
