Chinese Language for MBBS in China: Do You Need It? (2026)

Do you need Chinese language for MBBS in China? Lectures are in English, but HSK 4 and ward Chinese still matter. See the rules, timing and a study plan.

No, you do not need to know Chinese to get into an English-medium MBBS in China. Lectures and exams on these programmes are in English, and most universities do not ask for any HSK score at admission. But Chinese is not optional once you are in. It is a compulsory course, and almost every university sets an HSK 4 target before your internship or before your degree. Chinese language MBBS in China is really a question of when you learn it, not whether.

That is the honest answer to the forum posts claiming "professors teach in Chinese" and the agent claims that "no Chinese is ever needed". Both are half true. This guide explains what happens in the classroom, what happens on the hospital ward, which HSK level universities ask for and when, and a study plan that fits around a normal MBBS workload.

Is MBBS in China taught in English?

Yes, at the universities that run English-medium programmes for international students. China has a published quality standard for these programmes. The interim provisions on English-medium undergraduate medical education for international students ask universities to have teachers who can teach all subjects in English, and they require applicants to show strong English. So the classroom side is real: anatomy, physiology, pathology and pharmacology are delivered in English.

Two things are worth knowing before you believe every promise.

  • Not every Chinese medical programme is English-medium. Some are Chinese-taught and need HSK 4 or HSK 5 from the first year. Make sure the offer letter and admission notice say English-taught.
  • Some classes are still uneven. A Friday Times feature on Pakistani students in China reported that some professors lecture in Chinese and that students then rely on course outlines, assignments and library reading. This is more common in smaller or less internationalised departments than at universities with large international cohorts, and it is something to ask about before you pay a deposit.

The more serious language pressure arrives later, in the hospital. We cover that below.

Does the Chinese language still matter in an English-medium programme?

Yes. The same national provisions that require English teaching also say that Chinese "shall be a compulsory course during the whole undergraduate medical program in English". They list Chinese language and Chinese medical terminology among the courses students must pass, and they state the purpose: to help students with study and daily life in China and with contact with patients.

The wider Ministry of Education rule is simpler. The 2018 quality standards for international students in higher education (Jiao Wai 2018 No. 50) ask for at least Level 4 Chinese for undergraduates in foreign-language programmes. Universities then turn this into their own notice. China Pharmaceutical University, for example, writes it as "HSK Level 4 with a score of at least 180" for English-taught undergraduates, and says late or missing certificates mean the degree is postponed. Wuhan University adds that clinical medicine students "shall develop reasonable medical Chinese proficiency" on top of Level 4.

So the rule behind the headline is: English for teaching, Chinese for the national graduation standard and for the ward.

What HSK level do MBBS universities in China ask for?

Most English-medium MBBS programmes ask for HSK 4. The timing is what changes from university to university. Here are examples from our own university pages and from official university notices.

University or trackChinese language ruleWhere it comes from

Read the table as a pattern, not a promise. Rules change, and the same university can have an MOE-listed track and a non-MOE track with different language demands. Dalian and China Three Gorges both do this. Always check the rule on your admission notice, not on the pre-admission letter or an agent's brochure. Our China university directory lists each university's HSK rule on its page.

If you want the exam itself, the levels, passing scores and the new HSK 3.0 format that launches on 13 December 2026, read our HSK exam guide for Pakistani students. Official registration and results are on chinesetest.cn.

Will you need Chinese in hospital rotations?

Yes, in practice. This is where the forum critics are right.

Classroom English does not change what language the patient speaks. In most Chinese public hospitals, patients describe their symptoms in Chinese, doctors write the records in Chinese and ward rounds are held in Chinese. The national standard expects graduates to "communicate with patients and their families effectively". A 2023 study in BMC Medical Education on international medical students in China found that the language barrier with patients greatly reduced how much they learned in clinical practice.

What this looks like for you:

  • Years 1 to 3 are mostly lectures, labs and dissection. English carries you through, and Chinese is a side subject.
  • Years 4 and 5 bring clinical clerkships. Students who speak some Chinese can take histories, follow rounds and read charts. Students who do not often stand at the back and watch.
  • Year 6 is the internship. The national provisions set it at one year, at least 48 weeks. This is the year Chinese matters most, which is why many universities check HSK before it begins.

You will not become a fluent doctor in Chinese. You need functional ward Chinese: body parts, symptoms, pain descriptions, "how long", "where does it hurt" and the common instructions. HSK 4 is a reasonable base for that. It is not the end of the road, because a patient speaking fast in a regional accent is very different from an exam recording.

Can the internship be done outside China?

Sometimes. The national provisions allow internships at the host university's teaching hospitals or at approved affiliated hospitals abroad. Our Dalian Medical University page says HSK 4 is needed for an internship in China only and not if the internship is done abroad.

Do not treat this as a loophole until you have checked three things in writing:

  1. Your Chinese university confirms that it accepts an outside internship for your degree.
  2. The hospital abroad is approved by the university.
  3. PM&DC and the National Equivalence Board will accept your training record when you go for registration. Check this on our PMDC licensing guide and with PM&DC itself.

If any one of these is unclear, plan to learn the Chinese and do the internship in China.

Is there a language year before MBBS?

At some universities, yes. These are usually the English-medium Clinical Medicine tracks that are not on the MOE-listed MBBS list. In that format, Year 1 is a Chinese language year, and the medical subjects are taught in English from Year 2. China Three Gorges University and Dalian Medical University both offer a track like this next to their MOE-listed MBBS.

The trade-offs are clear from our fee data:

UniversityMOE-listed MBBS tuition per yearLanguage-year Clinical Medicine tuition per year
Dalian Medical University42,000 RMB25,000 RMB from Year 2
China Three Gorges University30,000 RMB24,000 RMB

The language-year route is cheaper per year and has more seats, and your Chinese will be stronger by Year 4. The downsides are that Year 1 has no medical content, the programme can carry a higher HSK demand (HSK 5 before graduation at China Three Gorges), and the degree title reads "Clinical Medicine" rather than MBBS. For Pakistani families, the more important question is recognition: check the university against the current PM&DC list before you commit. Our MOE-listed universities guide explains the difference between the two lists.

Do all universities in China teach in English?

No. Only a limited group of Chinese universities is approved by the Ministry of Education to teach clinical medicine in English to international students. Other programmes teach in Chinese and expect HSK 4 or HSK 5 before you begin.

A quick test before you apply:

  • Does the offer letter say "English-taught" or "taught in English" for your exact programme?
  • Is the university on the MOE list for English-medium MBBS? Look at our MOE-listed universities page.
  • Is it on the PM&DC recognised list for your intake year?
  • Does the university tell you in writing what its HSK rule is and what happens if you miss it?

If you are comparing countries because of language, our Russia vs China comparison covers a similar question for Russian programmes.

Can you fail because of Chinese?

You will not usually be thrown out for failing HSK alone. We did not find a university rule that dismisses students only because of HSK. But the documented consequences are serious enough.

  • Your internship can be blocked. Nanjing Medical University, for example, will not start an internship in China until the student reaches the required HSK level.
  • Your degree can be postponed. China Pharmaceutical University says degree conferral is postponed if the HSK certificate is late or does not meet the standard.
  • Your time can run out. Certificates must usually be submitted within the maximum study duration, and some universities will not extend your stay just to retake HSK.

A delayed degree means a delayed PM&DC file, a delayed licensing exam and a delayed salary. PM&DC does not ask for HSK. Your Chinese university does, and PM&DC needs the degree. Chinese can also affect your grades in clinical years, because ward assessments, case presentations and logbooks may involve Chinese. This is another reason not to leave it until Year 5.

How long does it take to learn enough Chinese?

A rough guide from our HSK guide, for regular study alongside other commitments, is 6 to 9 months for HSK 3 and 12 to 18 months for HSK 4. In China it is faster, because you are surrounded by the language every day. For HSK 2.0, HSK 4 means about 1,200 words. The new HSK 3.0 asks for about 2,000 at the same level number, so allow extra time if you test after 13 December 2026.

These are estimates, not guarantees. Students who study daily, speak to local students and canteen staff, and use a tutor tend to reach HSK 4 well before the internship. Students who only cram before the deadline often do not.

A realistic Chinese study plan for MBBS students

You have a heavy medical workload, so the plan has to be small and regular.

  1. Before you fly (3 to 6 months). Learn pinyin and tones first. Then aim for around HSK 1 or HSK 2. Even this makes your first month much easier: ordering food, using Didi, reading signs and talking to the hostel office. Our China arrival card guide will help you with the first steps at the airport.
  2. Year 1. Attend the compulsory Chinese class and treat it as a real subject. Aim for HSK 3 by the end of the year.
  3. Year 2 and 3. Move towards HSK 4. Book the exam at least two or three months before your university's deadline, because results take time and you may need a retake. Add medical Chinese early: body parts, symptoms, hospital phrases.
  4. Year 4 and 5. Use the wards. Ask a Chinese classmate or resident to help you take one short history each week. Keep a notebook of patient phrases.
  5. Before Year 6. Have your HSK certificate in hand and, if your university offers it, consider the Medical Chinese Test described in our HSK guide.

Practical habits that help: 20 to 30 minutes a day beats 4 hours on Sunday. Speak even when your sentences are wrong. Do not live only with Pakistani friends who speak Urdu all day. Use official HSK mock tests under timed conditions, and use a spaced-repetition flashcard app for characters.

What apps and courses help?

You do not need one perfect app. You need a mix of structure, vocabulary and speaking practice.

  • Your university's compulsory Chinese class is the base. Do not skip it.
  • Official HSK materials and mock tests from chinesetest.cn show you the real format.
  • Confucius Institutes in Islamabad (NUML), Lahore (University of the Punjab), Karachi (University of Karachi) and Faisalabad run Chinese classes and HSK centres. Check the current schedule with the centre.
  • A dictionary app with handwriting input, a flashcard app for characters and a podcast or video channel for listening are all useful. Pick what you will use daily.
  • A language exchange partner. Chinese students who want to practise English are everywhere on campus.

Titans Abroad adds each university's HSK rule to its university pages and can tell you which of your shortlisted options are heavier on Chinese before you apply, which is better than finding out in Year 4.

Questions to ask a university before you accept an offer

  • Which HSK level do I need, and at which stage: admission, Year 2, before clinical years, before internship or before graduation?
  • What happens if I miss the deadline? Is the internship delayed, the degree delayed, or both?
  • Is there a Chinese course in every pre-internship year, and is it counted in my grades?
  • Which languages are used on rounds and in clinical teaching at the affiliated hospitals?
  • Do international students have translators or bilingual supervisors on the ward?
  • Can I do the internship abroad, and will the university sign off on it?

Ask for the answers by email, not by phone. If a representative says "no Chinese is needed at all", that is a warning sign, because the national standard makes Chinese a compulsory course.

Frequently asked questions

Is MBBS in China taught in English?

Yes, at universities approved to run English-medium programmes for international students. Lectures, labs and exams are in English. The national standard still makes Chinese a compulsory course during the programme, and clinical years involve Chinese-speaking patients. Confirm that your exact programme is listed as English-taught on the admission notice.

Do I need HSK for MBBS?

Usually not to get admitted to an English-medium programme, but yes later. Most universities require HSK 4 before the internship or before graduation, and a few ask earlier. Some tracks need HSK 3 or HSK 5, and a few MOE-listed programmes list no HSK at all. Check your own university's written rule.

Will I need Chinese in hospital rotations?

Yes. Patients mostly speak Chinese, records are written in Chinese and ward rounds are often held in Chinese. English may work with some doctors, but not with most patients. Students who have basic ward Chinese by Year 4 learn far more from clinical rotations than students who start in Year 5.

How long does it take to learn enough Chinese?

For HSK 4, plan roughly 12 to 18 months of regular study, and faster if you live in China and practise daily. Medical Chinese for the wards comes on top. Start before you fly if you can, take the exam two or three months before your university's deadline, and keep studying after you pass.

Is there a language year before MBBS?

At some universities, yes. English-medium Clinical Medicine tracks at places such as China Three Gorges University and Dalian Medical University have a Chinese language year at the start. The MOE-listed MBBS tracks at the same universities usually go straight into medical subjects. Check recognition and the HSK rule for each track.

Do all universities teach in English?

No. Only a limited group of Chinese universities is approved to teach medicine in English to international students. Many other programmes are in Chinese and need HSK 4 or HSK 5 before you start. Check the offer letter, the MOE list and the PM&DC list before you pay any fee.

Can I fail because of Chinese?

You are unlikely to be dismissed for HSK alone, but you can be blocked from starting the internship or receiving your degree until you pass. Some universities will not extend your stay just for an HSK retake. That is why you should treat HSK as a deadline, not a suggestion.

What apps or courses help?

Start with your university's compulsory Chinese class, then add official HSK mock tests, a flashcard app for characters and regular speaking practice with Chinese classmates. Confucius Institutes in Islamabad, Lahore, Karachi and Faisalabad offer classes and HSK testing. Pick a small routine you can keep up for years.

Bottom line on Chinese language and MBBS in China

English gets you through the classroom. Chinese gets you through the ward, the internship and the degree rules. The Chinese language MBBS in China requirement is real, it is manageable, and it is much easier to meet if you start in Year 1 than if you panic in Year 5. Pick a university whose HSK rule you understand, get that rule in writing, and begin with pinyin before you fly.
