MBBS in China is cheaper than a private medical college in Pakistan only at the lower-fee universities. Five years of private tuition in Pakistan now costs about Rs 9.6 to 10.4 million before hostel and taxes. Six years at the three China universities we sampled costs about RMB 259,000 to 370,000, or roughly Rs 10.6 to 15.2 million. China also adds a licensing exam and a longer road to practice. This guide compares MBBS in China vs a Pakistan private medical college honestly, so your family can decide with real numbers.
We are an MBBS-abroad counselling service, so you might expect us to push China. We will not. For some families a Pakistani private seat is the better choice, and we say when below.
Quick comparison: China vs Pakistani private college
| Point | Private college in Pakistan | MBBS in China |
|---|---|---|
| Programme length | 5 years, then 1 year house job | 6 years including internship, then PM&DC steps in Pakistan |
| Tuition (sample) | Rs 9.6 to 10.4 million over 5 years | RMB 259,000 to 370,000 over 6 years at three sample universities, all fees included |
| Hostel | Optional, extra (Rs 168,000 a year at one college, Rs 42,000 a month with food at another) | Usually inside the fee table for the early years |
| Entry route | MDCAT and merit, or self-finance seats | University admission; MDCAT is no longer required for foreign admission since 5 October 2026 |
| Licensing after degree | PM&DC registration and house job | PM&DC registration, National Registration Examination (NRE), then house job |
| Language | English and Urdu | English classes, but Chinese needed with patients in clinical years |
| Distance from family | Same country | Flights, visa, winter |
Fees shown are from the 2025-26 schedules of two Pakistani colleges and from our own China university data. Treat them as samples, not a market average.
How much does a private medical college cost in Pakistan in 2026?
PM&DC capped private MBBS and BDS tuition at Rs 1.8 million per year from the 2025 session, covering lab, library and registration charges. From the 2026 session colleges may raise it by up to 5 percent a year, linked to inflation. A college can apply to charge up to Rs 2.5 million with full financial justification. Eduvision reported the cap in April 2025. Before it, some colleges charged up to Rs 3.5 million a year.
The actual schedules are the best guide. Two examples for the 2025-26 batch:
- Pak International Medical College, Peshawar: tuition Rs 1,890,000 in year one, rising to Rs 2,297,307 in year five. Five-year tuition is Rs 10,443,443, plus a Rs 37,800 admission fee and a refundable Rs 100,000 deposit. Hostel is optional at Rs 168,000 a year.
- Sir Syed College of Medical Sciences, Karachi: tuition Rs 1,750,000 in year one, Rs 9,618,000 over five years. Hostel with food is Rs 42,000 a month, and university charges, PM&DC fees and taxes are billed separately.
So the realistic range is about Rs 9.6 to 10.6 million over five years for tuition alone. Add hostel if you live away from home, university enrolment and exam fees, and tax. Many families end up near Rs 11 to 13 million in total. That is our estimate from these two schedules. Other colleges, especially in big cities, may charge more, so ask each one for its written five-year schedule.
Scholarships and instalment plans exist at some colleges, but terms differ. Ask what happens to fees if you leave in year two.
How much does MBBS in China cost for Pakistani students?
Costs vary a lot by university. On our MBBS in China pages, yearly tuition ranges from about RMB 12,000 to 75,000 across the programmes we list, and the all-in yearly total (tuition, hostel, visa extension and insurance) for most programmes falls between roughly RMB 19,000 and 60,000. Three MBBS examples from our university data, with rupee figures at about Rs 41 per RMB (the State Bank of Pakistan's daily sheet showed about 41 in June 2026):
| University | Year 1 (RMB) | Years 2 to 6, each (RMB) | Six-year total (RMB) | Approx. in rupees |
|---|---|---|---|---|
| Jilin University | 56,000 | 40,600 | about 259,000 | Rs 10.6 million |
| China Medical University | 68,000 | 52,600 | about 330,000 | Rs 13.5 million |
| Capital Medical University | 74,500 | 59,100 | about 370,000 | Rs 15.2 million |
Year 1 includes a one-time liaison fee of RMB 15,000 and a medical checkup. Some universities also charge a foundation course of about RMB 12,500 on arrival, and China Medical University students rent off campus from year 4, which is not in its table. Check the exact fee sheet on each university page.
Then add costs that never appear in a fee table:
- Food and daily life: our data puts food at about RMB 700 to 1,000 a month in northeastern cities, roughly Rs 29,000 to 41,000.
- Flights home, usually once or twice a year.
- Winter clothing and heating-related extras in the north.
- Exam attempt fees and preparation for the NRE after you return.
- The year or more you spend before earning, compared with a local graduate.
Rupee-yuan rates move. A weaker rupee raises your cost during the six years, and a stronger one lowers it. Our own planning guide on self-funded MBBS abroad costs compares all four destinations side by side.
So which one costs less?
It depends on which China university you compare with which Pakistani college.
- A lower-fee Chinese university with yearly totals near RMB 20,000 to 30,000 is clearly cheaper than a Rs 9.6 million private seat, even after flights and food.
- A mid-priced one like Jilin lands around the same as a private college.
- A higher-priced one like Capital Medical University costs more than most Pakistani private seats.
The straight answer is that China is not automatically the cheap option any more. The PM&DC fee cap narrowed the gap that used to exist when some private colleges charged up to Rs 3.5 million a year. China still wins on price when you pick a lower-fee MOE-listed university. Check which universities are on the MOE-listed universities page and read our full list for Pakistani students.
How long does it take to become a practising doctor?
Pakistan private route: five years of MBBS, then PM&DC provisional registration, then one year of house job at a PM&DC-approved hospital. The PM&DC Registration Regulations 2023 require that year for full registration. That is six years if all goes to plan.
China route:
- Six years in China. Universities such as Nanjing Medical University describe the MBBS as six years including a one-year internship.
- Apply to PM&DC for registration of your foreign degree.
- Pass the NRE.
- Complete the house job in Pakistan.
That gives seven years at the very least, and more if you do not clear the NRE on the first attempt. Our PMDC registration guide for foreign graduates walks through each step.
What is the NRE, and how hard is it?
The National Registration Examination is PM&DC's licensing test for foreign-qualified graduates. It has two parts, and candidates must score at least 60 percent in each part separately, according to a report on the June 2025 session. Reports in earlier years gave a different pass mark, so check the current notice on pmdc.pk before you plan around it.
The pass rate is the most important number in this whole decision. Result.pk reported that in the December 14, 2025 session about 21.17 percent of medical candidates passed, out of 7,076 registered, and the June 2025 session had a 25.26 percent medical pass rate. These figures cover graduates from all foreign countries, just China, and we found no published China-only breakdown from PM&DC. One news report put China at 333 passes from 2,154 candidates in the December 2025 sitting, about 15 percent, but we could not confirm that from an official source, so treat it as unverified.
What this means for you: a foreign degree does not guarantee a Pakistani licence. Students who study seriously from year 1 for the NRE syllabus and work through Pakistani-style questions during clinical years do much better than those who plan to start after returning. Ask any university or agent how its Pakistani graduates performed, and ask for evidence, not stories.
PM&DC also states that only graduates of institutions recognised by ECFMG can sit the NRE. Confirm your target university's status in ECFMG's directory, plus its PM&DC status, before paying anything. We covered the topic in our licensing guide.
What about MDCAT and entry rules for China?
MDCAT is still required for medical and dental colleges inside Pakistan, but not for admission abroad. PM&DC Public Notice No. Exam-59/2026/350, dated 5 October 2026, says its Council decided on 14 September 2026 to exempt the mandatory MDCAT requirement for Pakistani students seeking admission into foreign medical and dental institutions. Still required are the National Examination Board (NEB) assessment to transfer into a Pakistani institution and the registration exam (NRE) for foreign graduates who want to practise in Pakistan. Only graduates of PM&DC-recognised institutions get provisional registration and can sit the NRE.
One point is unconfirmed. PM&DC's earlier May 2026 notice also listed FSc (Pre-Medical) at 60% or more, online registration on srf.pmdc.pk and training conditions such as hours, attendance and a local language course. The October notice does not say whether they still apply. Confirm with PM&DC at pmdc.pk before enrolling. Our guide on whether MDCAT is required for MBBS abroad explains the current position and how to verify it.
For students with low merit, China is often the realistic choice. If your MDCAT score does not reach a public seat and the private fee is out of reach, you can look at lower-fee China universities. Just confirm the remaining PM&DC conditions first.
Language, clinical training and daily life
Most MBBS programmes for international students in China teach in English, and universities add Chinese language courses. Xi'an Jiaotong University's international programme describes a first stage focused on Chinese language and medical terminology. In hospital wards, many patients speak only Chinese. Students who skip Chinese study find the clinical years hard, and some of the extra-year stories you read on forums come from this.
Daily life is manageable but different. Northern winters reach minus 15 to minus 20 degrees in cities like Shenyang and Changchun, according to our university data, though buildings are centrally heated. Our university pages list halal restaurants near the campuses we cover, but food takes time to get used to.
In Pakistan you train in the same language your patients speak, and your clinical exposure is in the system where you will probably work. That is a real advantage for private college graduates when preparing for house job and postgraduate exams. China-trained doctors usually see fewer of the disease patterns and hospital workflows common in Pakistan, and they must make up that gap themselves.
When a private college in Pakistan is the better choice
Pick the Pakistani seat if:
- You can pay Rs 10 to 13 million without selling your only house or borrowing at high interest.
- Your merit is good enough to get a seat at a college with a solid teaching hospital.
- You want the simplest route to a licence, with no NRE.
- Your family wants you close, especially for a daughter or a younger student.
- You are not comfortable studying and living in a different language for six years.
Pick China if:
- Your budget only stretches to a lower-fee MOE-listed university, where yearly totals are far below a private seat.
- Your merit does not reach a good local seat and the private fee is out of reach.
- You are willing to study the NRE syllabus early and take the licensing exam seriously.
- You can handle distance, cold weather and a new language.
A third option for many families is a public medical college seat, which costs a small fraction of either route. Keep that door open as long as the rules allow. Those with scholarships should also see our guide to fully funded MBBS scholarships, because a funded seat changes the cost comparison completely.
Break-even logic for your family
Here is a simple way to compare. Take your Pakistani college's written five-year schedule and add hostel, university charges and tax. Take the China university's six-year fee sheet, convert at today's rate plus 10 percent for exchange risk, and add food, flights and an NRE preparation budget. Then add the cost of the extra year: a Pakistani graduate starts house job earlier than a China graduate who must first clear the NRE.
If the China total is lower by more than about Rs 2 to 3 million, the saving is probably worth the extra steps. If the gap is smaller than that, the Pakistani seat is likely the better value for most families, because you remove the NRE risk and the extra year. This threshold is our own rule of thumb, not an official figure. Adjust it to your family's risk tolerance.
Where Titans Abroad fits in
Titans Abroad is a free counselling service for Pakistani students applying for MBBS in China, Russia, Georgia and Azerbaijan. Titans Abroad does not charge a consultation fee, and we only recommend PMDC-approved universities, MOE-listed ones for China. If a Pakistani seat suits your profile better, we will say so. You can start through our application page, or read how the complete MBBS in China guide handles admission step by step. Be careful with any agent who guarantees a seat, asks for cash up front or hides the PM&DC steps. Our guide on spotting a fake consultant lists the warning signs.
Questions to ask before you decide
- What is the full written fee schedule for every year, with all extras?
- Is the university on the MOE list, recognised by PM&DC and eligible under the ECFMG rule for the NRE?
- How many Pakistani graduates cleared the NRE, and in how many attempts?
- Where will I do clinical rotations, and in which language?
- What is the refund policy if I do not get a visa or want to leave?
- Have I checked with PM&DC whether student registration is still required before enrolment?
Our view: MBBS in China vs a Pakistan private medical college
For most families the choice comes down to two numbers: the gap in total cost and your honest chance of clearing the NRE. If a lower-fee China university saves you several million rupees and your child will study seriously, China is a sound plan. If the saving is small, the private college in Pakistan is simpler and safer. Whichever you choose, get every fee in writing, verify recognition on pmdc.pk, and plan for the licensing exam from day one.
Frequently asked questions
Is MBBS in China cheaper than a private medical college in Pakistan?
Sometimes, but not always. On our fee tables, three sample China universities cost about RMB 259,000 to 370,000 for six years, roughly Rs 10.6 to 15.2 million at Rs 41 per RMB. Five years of private tuition in Pakistan runs about Rs 9.6 to 10.4 million. Cheaper Chinese universities beat that. Mid-priced and expensive ones do not.
Which is better, a Pakistani private college or China?
Neither wins for everyone. A Pakistani private seat is simpler: one country, no licensing exam for foreign graduates, and a direct path to house job. China can cost less at lower-fee universities and gives international exposure, but adds the NRE, a longer timeline and a language gap in wards. Choose by budget, merit and how much risk your family accepts.
How long does it take to start practising after China MBBS?
Count at least seven to eight years from the day you fly. The China programme is six years including internship. After that you apply to PM&DC, sit the NRE, and then complete a house job in Pakistan, as the PM&DC regulations require one year of house job for full registration. Failed NRE attempts add months or years.
Do China graduates have to pass an exam in Pakistan?
Yes. Pakistani nationals with a foreign primary medical degree must clear the PM&DC National Registration Examination (NRE) to get a licence. PM&DC states that only graduates of ECFMG-recognised institutions are eligible to sit it. Check your university against that rule before you pay a deposit, and read current notices on pmdc.pk.
Is a Pakistani private degree more respected?
Once you hold full PM&DC registration, the licence is the same whichever route you took. Forum users do mention bias against foreign graduates, and the low NRE pass rate feeds it. The real difference is the extra steps to reach registration, not the paper itself. Respect in practice comes from your training and your house job record.
What if I get a public seat later?
A public medical college seat is the cheapest and strongest option, so keep trying MDCAT if you are still eligible. If you have already paid a China deposit, ask the university for its refund terms in writing first. PM&DC also holds a separate National Equivalence Board exam for foreign graduates, so check pmdc.pk for who can sit it.
How much is the private medical college fee in 2026?
PM&DC capped private MBBS tuition at Rs 1.8 million per year from the 2025 session, with up to 5 percent increase linked to inflation from 2026. Colleges can ask to charge up to Rs 2.5 million with justification. Published 2025-26 schedules show about Rs 1.75 to 1.89 million in year one, before hostel, university fees and taxes.
Which option is safer for a daughter?
A Pakistani college near home is easier for most families to supervise, and that matters. China means distance, cold winters and a new language, and your daughter will be far from family if something goes wrong. Read our safety guide for parents, ask the university about female hostels, and speak to current Pakistani girls on the campus.
